For builders Playbook 02 · Page 2 of 3

If you want in now, here is exactly what you do.

Eight lines, in order, with the cost and the clock on each.

Page one is what he did. This page is what you do. It is short on purpose: read it once, fill in the gaps yourself, and start the two slow clocks this week.

Forma has no connection to MEDVi. Every figure here is reported by the New York Times, Forbes, the FDA or Meta’s public ad library, and is linked at the bottom of each page. Nothing in this guidebook is advice to sell medicine.

The sequence

Eight lines. Fill in the rest yourself.

  1. Pick an open lane.Brand-name GLP-1 at cash price plus your fee; 503A with a clinical reason and a lawyer; or the same machine on testosterone, ED, hair, menopause, longevity. Mass compounding closed in February 2025.
  2. Book the OpenLoop call today. They supply clinicians in 50 states, prescriptions, pharmacy, intake, dashboard. Custom quote; you set retail above their floor and keep the gap. Longest clock you control.
  3. Site in a day, with AI.Landing, three questions, verdict, price, handoff to the partner intake with the plan in the URL. Their intake is your checkout. Build nothing else.
  4. Freeze the copy.No “FDA-approved”, no “compounded”, no outcomes, no before-and-after, no testimonials you cannot prove. Every edit during review restarts a clock.
  5. File LegitScript, expedited.$975 + $2,150 a year, +$2,500 to expedite. Three days to an analyst, then two to six weeks. Answer each question round the same day.
  6. Meta, in this order.Business verification, domain verification, payment method, two weeks of warm-up with no health ads. Then, with the certificate, the prescription-drug permission (US, CA, NZ, 18+). Accept the Custom Audience terms.
  7. Ads: page three.Twenty on day one, kill in two weeks, keep two, your own face. The rest is on the next page.
  8. Calendar and cash.Eight weeks and under $10,000 to the first paying patient. Recurring revenue starts the day the certificate lands, because the partner already bills monthly. Start the two slow clocks in the same week.

Nothing here needs a licence, an employee or an investor. It needs two forms filed in week one and copy you do not touch.

Reality check, September 2026

The ocean is still large. It is much smaller than his.

He had a shortage window, a $149 price nobody else was saying, and a Meta feed with no competitors in it. This month the Ad Library shows about 580 active ads saying “tirzepatide” and “$149” together, a price floor of $89 to $99 for semaglutide, agencies selling his playbook as a course, and his own GLP-1 page nearly dark while he sells ED. Demand is real and growing. The margin per patient is not what it was. If you still want in, plan on these numbers.

LinePlan onWhere it comes from
Cash before the first patient$8,000 to $12,000LegitScript $975 + $2,150 + $2,500 expedite; entity, domain, email, tools under $1,000; two months of test ads.
Ad spend to learn anything$1,500 to $3,000 a month for two monthsTwenty ads at $50 a day, the two-week kill rule, one re-upload wave. Less than this and you cannot tell luck from a winner.
Cost of a paying patient$150 to $250 on a new accountHealth CPMs of $40 to $70, a few percent of clicks finishing the partner intake and being approved. Scaled accounts with proven creative do better; you are not one yet.
What a patient pays$149 to $199 a month, or $1,400 to $1,900 for a prepaid yearThis month’s feed. Above $199 monthly you are the expensive one.
What you keep$45 to $60 a month on a monthly plan; $250 to $550 up front on a prepaid planRetail minus the partner’s floor for clinician, pharmacy and shipping. The partner keeps roughly two thirds of a monthly plan. This is why every ad pushes the long plan.
How long a patient staysThree to five months, medianCategory churn; side effects, cost and goal reached. Net lifetime on a monthly plan is about $200. On a monthly-only mix you buy patients for what they are worth.
Month three at $3,000 of ads12 to 20 new patients; $1,500 to $4,000 kept; about break-evenCAC against net revenue. Profit at this size comes only from prepaid plans and from patients who stay past month four.
Where it becomes an income$20,000 to $30,000 a month of ads, CAC under $180, a third of sales prepaid: roughly $8,000 to $15,000 a month to youTwo hundred new patients a month, a few hundred on file. Two people can run it. It is a good business, not his business.
  • The way out of break-even is the prepaid plan. A $1,807 year keeps you about $550 the day it is sold, three times the lifetime of a monthly patient, for the same ad cost.
  • The second way out is a lane with no $149 floor. Men’s health and longevity carry $130 to $200 of margin per order with the same partner, and this month’s feed shows where he went.
  • The way in is still eight weeks. Nothing above starts until the certificate lands. Everything above is planning numbers from a signed partner schedule and this month’s Ad Library, not a promise.

At the foot, as a warning
    1. And it shall come to pass, if thou shalt hearken diligently unto the voice of the LORD thy God, to observe and to do all his commandments which I command thee this day, that the LORD thy God will set thee on high above all nations of the earth:
    2. And all these blessings shall come on thee, and overtake thee, if thou shalt hearken unto the voice of the LORD thy God.
    Deuteronomy 28:1–2
    1. Is not this the fast that I have chosen? to loose the bands of wickedness, to undo the heavy burdens, and to let the oppressed go free, and that ye break every yoke?
    Isaiah 58:6
    1. For I was ashamed to require of the king a band of soldiers and horsemen to help us against the enemy in the way: because we had spoken unto the king, saying, The hand of our God is upon all them for good that seek him; but his power and his wrath is against all them that forsake him.
    2. So we fasted and besought our God for this: and he was intreated of us.
